SSS Requirements for ID Registration
To apply for an SSS ID in the Philippines, you need to provide several documents to prove your identity and eligibility. The main requirement is an authentic and valid primary ID.
However, if you do not have a primary ID, you must present two valid secondary IDs. Now, let me discuss, what these primary and secondary IDs include. The list is as follows:
Primary ID Includes:
- Unified Multi-Purpose ID (UMID) Card or previous Social Security Card (for replacement or upgrade)
- Driver’s license
- Philippine passport
- Postal Identity Card
- NBI clearance
- Alien Certificate of Registration
- Seafarer’s Identification and Record Book (Seaman’s Book)
- Firearm Registration
- Permit to carry firearms outside residence
- Firearm Ownership and Possession License
Secondary ID Includes:
- Pag-IBIG Member’s Data Form
- TIN ID
- Police clearance
- PhilHealth Member’s data record (MDR)
- School or company ID
- Birth certificate
- Academic transcript
- Senior citizen card
- Marriage contract
- OWWA card
- PRC license
- Barangay/Municipal/City ID
- Fisherman’s card
- Certificate from the Office of Indigenous People
- Seafarer’s Registration Certificate

If you don’t have any of the primary IDs that I listed above, you can present two valid secondary IDs with your signature. Also, make sure that one of the secondary IDs includes a photograph of yourself. By gathering all the relevant details, you can apply for the Social Security System (SSS) ID.

SSS Requirements for New Member
When registering with the SSS for the first time as a prospective employee, complete SSS Form E-1 (Personal Record) and provide an original or certified true copy, along with a photocopy of one of the following documents:

- Seaman’s book
Surviving spouses and guardians aged 60 and above, applying for an SS number under the Annual Confirmation of Pensioners (ACOP) may submit secondary documents with a recent photo and age. The secondary documents are listed below:

- Alien Certificate of Registration
- If the ATM card does not show the cardholder’s name, include a certification from the bank confirming that the account number belongs to the cardholder
- Bank account’s passbook
- Birth /Baptismal certificate of children

- Certificate of Naturalization issued by the Bureau of Immigration
